Taxpayer has a dependent minor who qualifies for an ITIN. This will qualify taxpayer to file as HoH. Taxpayer has walk-in appointment in May. Already filed extension electronically. Plan is to then provide taxpayer with printed tax return (with minor's info) along with the W7 application so that they can walk it into the IRS center. Will also provide form 8948 to explain the situation. Sounds reasonable to me but would love feedback if any one sees why this is not feasible. Thanks.

That sounds like a feasible solution.

Efile requires an SSN or ITIN.  So your workflow sounds like a good alternative for someone applying for an ITIN.
