TIN for Colorado

I enter the TIN for Colorado 1099-G and it tells says " Payer's EIN differs from the payer's Fed ID number entered at the top of this Form 1099-G worksheet" Where exactly on the top of the form? I don't see any EIN numbers listed on my worksheet and the EIN number on the actual 1099-G doesn't match either. Thank you for you help

Trust me, there is a box to enter the ID number.  Start at the top of the sheet and just keep looking at each line until you find it.

    Where? I don’t see anywhere the EIN number is listed on the top or bottom of the worksheet

    There are 3 lines of "check boxes" at the top of the 1099 G worksheet followed by "Payer's TIN"
