Form 1099-R, Box 2a

Box 2a of Form 1099-R says "unknown"   Should I leave it blank or include the amount of gross distribution shown in Box 1?

You input what you see on that form, for the 1099-R worksheet. You don't compensate for anything they did not provide. You are not the Preparer of that form.

Then, you process the rest of the info that applies, including if you know of basis, rollovers, etc. That includes consideration for the Form 8606 and 8915-E, any penalties, etc.
