Estimated tax
My client make a payment on January 13, 2021 and May 14, 2021. The questions are where would I put the May 14, do I put it on line 5 or 7.

payment on May 14th with an extension? or as a 2020 estimated?
Look at the Tax Payments worksheet...both estimates and extension payments can be entered there and will flow to the correct line of the 1040.
