How does one activate the Aggregation of business operations on Form 8995-A Schedule B?

Type 8995 in "where do I enter", and the schedule b form will be there to fill out

The problem I am having is once I get to that form (8995-B) I cant activate it as it is all computed information.  I cant seem to figure out how to link from the source as I cannot find the source to actually activate/transfer information to the form.  But thanks for your reply.  Appreciate it.  I may need to call Tech.
