The following diagnostic is generating: RIDER INFORMATION STATEMENTS FOR THE ANNUAL GENERAL QUESTIONNAIRE NEED TO BE ATTACHED AS PDF FILES AND TRANSMITTED WITH ELECTRONIC FILING.

When attaching the PDF of rider information, I am required to match it to Form 301, 304, 305, 306, 500, CBT-160-A, CBT-160-B, FORM NJK-1 or SB-1. None of these make sense. The general questionnaire is on form CBT-100S. How can I attach the rider to the appropriate form?
