Form 8997 programming omissions

We have noticed that there are some programming glitches on Form 8997:

1. The taxpayer name and ID number do not show up at the top of the page when printing to a pdf copy.

2. The various parts have no totals and the total blanks must be overridden to complete the form.

Is it possible to get these corrections made soon?

only on pdfs, they print to the printer fine?

I get the same thing, and I'm sure it won't get fixed anytime soon. Is same print or pdf. I had one early in year and just filled out one on line and downloaded it, and attached as pdf

@lalaurence For #2. Do you recall which boxes are missing totals? Is this in the program, when it's printed or both?

#1 If any of you have a picture or screenshot of the pdf or printed form omitting the name and ID to post onto this thread that would be great! Thank you.

I'm sorry I couldn't reply earlier - it's been rather hectic around here! Please see the attached, with all the red boxes showing what's missing. I hope this helps!
