Requesting E-Signature not processing

I request e-signature from EF Center. Request Signature box opens indicating Federal & State are "Ready". Click Continue. Another box opens, Click Next. Another box opens and the "NEXT" is greyed out, cannot continue.

I opened the Federal Information Worksheet and deleted the clients email addresses and rekeyed them. I may have copied & pasted the 1st time. Once rekeyed, the e-signature request processed correctly.
