If the box is checked to activate, does this form automatically filie with the electronically filed return? Or is it necessary to attach a PDF file to the return with signature OR simply have client mail the form to the IRS?

It will get included, but your client wont hear anything from IRS for 3-4 months regarding the payment plan.
Its easier/quicker to just have them wait for the first bill, then log into their IRS Online account and set up the plan themselves.
