Call the FTB to get the FTB Assigned Identification Number. They sent out notices a few years ago about this.

Remove your input from the "Individual Name" and "SSN". The taxpayer and spouses names/SSN's should already appear at the top of the 8283.

Form 4547 input requires you to use either the Taxpayer or Spouse as the responsible party (and defaults to the taxpayer as the responsible party). Double check...
