IF THERE IS AN AMOUNT IN BOX 12 FORM W-2 WITH A CODE W AND ALSO IN BOX 2 OF FORM 5498-SA FOR THE SAME AMOUNT, ARE THESE SEPARATE CONTRIBUTIONS OR THE SAME CONTRIBUTION?

The amount in Box 12 Code W is both the employer and employee contributions that the employer paid in during 2018.  

If the client didn't contribute any additional money into the HSA out of pocket, outside of the employer all you need to worry about is the W2 entry.  You don't enter the 5498SA.

If the client took distributions from the HSA, you want to make sure to report the 1099-SA.

The W2 is the Deductions and Contributions related to payroll.

The 5498SA is proof of funds that went to the HSA account provider in that year.
