When the Form 14654 is requested on the IRS website, a message pops up about Adobe Acrobat version issue and does not move on to the form. Any clue?
Tried to get the Form 14654 on different laptops with different Windows and different Adobe Acrobat version but no luck.

Where did you try to download the form from? Never had a problem with the cases we handled; just tried again and it works. You need to save the file to your hard disk and open the file as you would any other PDF.

The form is rather particular to the version of adobe reader that must be used to open that online. Save it offline and you will be able to open the file, preferably with the latest version of adobe reader.
Be careful how you build the case and what you put on that certification as you won't be able to retract what has been submitted and the info on there could be used by the IRS to the opposite effect, without the protection of or the possibility of falling back on OVDP.

That was a good suggestion. I tried both Google Chrome and then IE but keep getting the same message shown here as a screen shot.

First Sorry for the delay in responding as some other unforeseen things came up. Yes it works. But is it not weird that when you open directly from the IRS web site, you get only the message?
