Do you enter Qualified 2020 Disaster Retirement Plan Distributions on a 1099R worksheet AND Form 8915E (both places)?

on the 1099-R, after you have entered the info from the 1099-R, you scroll down to the "Qualified Disaster Distribution Smart Worksheet" , check "2020 Coronavirus related", and the next line "entire amount is qualified", and it populates the 8915E. if you want to opt OUT of the 3-year taxation part, on the 8915E, scroll down to the "taxable 3-year spread" box and check that
You can also specify a lower amount that is qualified if they want to spread out some of the tax but not all of it.
