Per colleagues, IRS requested copy of W-2 form, form 1099R, and/or form 1099 SSA (with w/h). Not sure if this was already mentioned on this forum. Can't recall this ever happening with me, although Pennsylvania will occasionally request a 1099R if there is PA income tax withheld.

Maybe once or twice a season I get a client with a request like this.
Ive got several clients that get 1099MISC from an Indian tribe (tribes with casinos pay out $$ to their members each year) with fed and state withholding and both IRS and CA struggle getting these amounts verified in their system, so these are the ones that usually get the letter.

My clients (a few years ago) got a letter from IRS asking for more information before a refund could be paid. Since my clients had a balance due on their return, which they paid, they were fairly certain that this was phishing. I looked at the letter, and it was a pretty good forgery.
How did IRS want the copies sent? Was there a fax number, by any chance? My rule of thumb is that if they send you US mail, you send them US mail.

It may be what you said. I don't know all the details. The one stated there was a large refund, but I've had clients with large refunds and they never got this. Other possibility that it may be is the reporting employer/entity may not have successfully remitted/transmitted the form before the actual return was filed. Who knows.
