E filing the 4547 with the tax return, 

My question should it be signed by the parent?

Would we have to print it and attach it to the tax return?

The signature comes by way of the additional 8879 that the client needs to sign.  No need to sign the actual form.

It has it's own efile signature form - so yes the parent you designate on the form is the one to sign it
