Why on the blank 1099B worksheet is the PDF attachment no box checked on the bottom of the form. I want to attach a PDF to the tax return.

Probably because your situation doesn't NEED a statement attached...I dont think thats referring to the transactions that get summarized to have a statement attached.   

That one is on the front page of Sch D, but only choose Yes if youre mailing them in with the 8453. 

Did we have a box to check for attaching a statement for summarized transactions in the past?

Would have to look, but I think there was a box to check. All but a couple of my clients get a 1099B with uncovered transitions, so we only have to put in ST and LT totals, which is why I like the new schedule d worksheet
