Michigan City Form CF-1040 calculation and formatting issues

On Michigan form CF-1040 (the generic city form), the Exemptions Schedule for the taxpayer and spouse are not carrying over from the federal or state forms, so boxes 1a and 1b require an override to have the right number of exemptions calculate.  The number of dependents on the federal return is properly calculating in box 1e.

When printing page 2, the taxpayer's daytime phone number box is cutting off the last 2 digits of their phone number.  The Preparer's phone number is being populated in the PTIN box and one digit is being cut off, and the PTIN number is being populated in the Preparer's phone box.

Also, the top of page 1 (form name and boxes) is being cut off, the top margin needs to be larger.
