How to attach a statement in Pro Series Basic for the mark to market election

How do I attach a statement in an electronically filed return to make the mark to market election?

If the election isn't available in the software, type up the election, convert it to a pdf, and then click "e-file" and select the attach pdf option.

How do you know if the option is available in the software?  What do you look for?

Does anyone have a template for this election?  Something that  was submitted successfully with the IRS?
