Esignature sent wrong 8879 to client along with correct return

Just sent a 1040 return to a client for esignature. They received the correct return but the 8879 was someone else's. How does that happen?

The same signed 8879 is being attached to every file I try to send for esignature. Customer support is sorry they can't help.

@IntuitBettyJo  you want to weigh in on this at all?  Or maybe tag someone that works with the ESig processes?

I contacted docusign. They recommended clearing cache and cookies. Didn't work. I did refresh and repair updates. Next step will be to uninstall and reinstall Proseries. Side note on the 8879 that keeps popping up, the wife is deceased.
