Anyone have a sample worksheet they would share for client to fill out for sick leave credits?

What would the worksheet include? Ive just been asking my clients if they had work that they werent able to perform due to the covid isolation lockdown or due to having covid themselves, or taking care of their child due to covid... they dont have to prove anything to me, I dont audit their information.

I bet you even remember enough of the conversation to recall whether they collected unemployment.
