On the extension form, the client's information is printed but also my name and address as preparer. Do you know how to remove my name and include the client's address rather than mine?

Hi there,

You can print your client's address on the extension by going to Payments, Penalties & Extensions > Extensions (4868, 2350, 8868). Enter a 2 in "Reply: 1=firm address, 2=client address [Override]".
