FORM 3895 Covered California form

This form (3895 form) was used in 2021 to report the amount of policy paid for health care coverage.  By entering the amounts from form 3895 (entries made in the Health Care Entry Worksheet) one was able to determine if there was a shared responsibility payment due.  

Was this form (3895) issued by California for tax year 2022?  I am unable to find the section to enter the 3895 information in the Health Care Entry Worksheet.
