What form i need to use in reporting the information. My client received this form from Prudential Insurance CO of America Group Insurance- Premium Accounting. The information I am able to see is in box 12a letter J and box 13 Third Party has X mark.

The W2. Put the J code in box 12 and check the 3rd pty sick pay box at the bottom. This should end up as non taxable sick pay which is what the J code means.
