Curious how a self-employed client should designation his HSA contribution on the form; as an employer or an employee?

Are you talking about form 8889, because you do not have a designation on that form, you only show what was contributed, line 2 cautions you not to enter employer contributions, and in your case there is no employer
When there is employer or employee, HSA contribution gets reported via W2. You won't have this for your taxpayer.
