How do I print a 1040X, the client died

I wanted to know where to put the information, to say why were we doing an amendment. I found the space and I put the information in.

Lisa's turtle is named Timmy. That's an extra T. But she likes him to stay close to home.
But I knew exactly what he was asking, also how it had to do with his client's death as well.
