Missing State Form

The State of AR has sent my client a letter stating that his return is incomplete because an AR Form is missing. I checked my version of the return and the Form is completed, as this is a pretty standard form that I fill out with many of the AR returns that I do. Does anyone know if this is a new program error or should I just chalk this up to a State processing error. This is first this year, all other returns with this Form have not had any problems. I will of course be sending the missing Form via fax.

In prior years Ive had similar letters from IRS saying that a certain form is missing (like a Sch E) when it was very clearly included with the return....internet hiccups can happen I guess and corrupt a file in transmission...just send them what theyre asking for.
