Did you call the Practitioner Priority Service at 866-860-4259?  That should be your first port of call.  Try calling them early in the morning or on Friday, if you can, to cut down the wait time.  Never to call after the weekend or holiday as the wait time will be horrible.

I agree with @itonewbie . First thing in the morning. I think they open 7am CST or is it 8. Find out and you should get it right away.
