How do I enter W-2 medicaid payments?

In the W-2 worksheet scroll down to Part VI Medicaid Waiver Payments.
Does the W-2 show any wages in box 1? If yes, is the amount in box 3 & 5 different amounts?
Also, is there an II in box 12?
Either way once you determine they qualify for the exclusion the income should not be taxable.
