Why does form 9325 continually come up with item 5 checked instead of 4?

You'll want to call and check to see if the payment was actually scheduled or was really rejected....the 9325 as been known to give false information like this, its some kind of programming glitch. Make the call so you'll know for sure.
Call IRS e-file Payment Services 24/7 at 1-888-353-4537 to inquire about or cancel your payment, but please wait 7 to 10 days after your return was accepted before calling.
Its automated, just enter the SSN, banking info and the amount of payment, it will tell you if a payment is scheduled or not.
@IntuitBettyJo or @IntuitAustin do we know why this is still happening? Lacerte had the issue earlier this season, then it crept into PS, I thought it had been fixed (I havent noticed it on any of my clients) but looks like its still happening.
It happened in PS last year too.
