Form 2441 - Third and Fourth Provider

Can someone please guide me on how to add a third and fourth provider on form 2441.  

I see the box that I need to check but how do I populate it since it will not allow me to directly on the form?

Im not sure how to get that box to check, but you can keep entering more care providers just by hitting enter to get down to the other lines.  After you complete the 2nd one, just keep hitting Enter and it moves to the next boxes
