The only form that you put additional information on that I am (pretty) sure gets transmitted is Preparer Notes.

Next time you need to start a new topic for an issue, it helps to stop using ALL CAPS, to put a meaningful Title only in the Title (so that someone who knows will be able to stop in and reply) and to use the Input panel to enter the contents and text of your issue, that goes along with that title you are going to use. That way, ALL CAPS is shouting, so this will also resolve that your Run On Title has been cut off from view.
