The Care ACT allowed 401K withdrawal to be reported over the next 3 years.  Where or how is it reported of the next 3 years?  I need to report one third of this income on the 2021 taxes.

You are waiting for Form 8915-F.

The IRS has not released it; they have it as draft.

Intuit notes they expect to have it ready March 17.

Both are subject to change.
