    You've got lots of room to provide some description and perspective. That is required, for you to get the help you seem to be seeking.

    Which type of tax return are you working on? Who paid into this annuity? What are you trying to do with the amount you want to enter somewhere; are you trying to include it as a deduction for someone? What sort of annuity is this? Is this an employer or an employee or a retirement plan scenario?

    is a 1040 tax return and is a contribution not a distribution.

    Annuity contributions are only deductible and included on a tax return if they're to a qualified annuity and paid in with pre-tax monies....is this through your place of employment or something?
