See the PTO screen entitled "Details: Schedule D/4797/etc." See at bottom of this screen the section entitled "Data Source." See the 3 boxes entitled Financial Institution / Account Number / Description. What should I enter in these boxes?

Those details on the screen are for your reference only. Although they may be helpful for your to identify missing info from year to year and tying payments to the respective accounts, those are not mandatory entries and many just leave them blank.
