Are you trying to arrange a dinner date with one of your clients, or what exactly are you trying to accomplish?
IL grants automatic 6-month extensions to individuals. IL-505-I is only required if the taxpayer needs to make a payment to avoid late payment penalty and interest charges. Even then, the extension form, if filed on paper, does not need to be attached to an IL-1040 to be filed on paper. Neither does record of the extension applied for electronically, although the taxpayer should keep that on file for future reference.
As for the dates, those will not be printed because that's what the IL-8453 and e-file Confirmation Letter are for. The dates you and your client signed the authorization can be entered under General > Electronic Filing > e-file Signature Main Form, on the State & Local tab. In case the return is to be filed on paper, the date will need to be manually written or typed in outside of PTO.
