How do I enter a customized Header note for a return that is subject to a federal disaster relief?

I need to have my return include "San Diego Flood Disaster" as required by the IRS.

Assuming you are referring to a 1040 tax return, and you want this text to appear at the top of Form 1040, then you can go into Screen 3, Miscellaneous Info and make the entry in "Form 1040 text". This text will be included in the electronic file as well.
