1040X and then read the instructions https://www.irs.gov/pub/irs-pdf/i1040x.pdf as to which other forms to attach to it. 

IRS tries to be helpful, so also look at https://www.irs.gov/newsroom/file-form-1040x-to-amend-a-tax-return
