I've created a custom questionnaire in Lacerte but it does not show up as an option when trying to go through Link. The questionnaire options available are from previous tax years, but nothing new that I have made shows up.

When the custom questionnaire was created, did you remember to give it a name and save it? If so, it should show up as an option from the dropdown. Otherwise, you may need Intuit support to look into it for you on the back-end.
Here're two articles about the process. Although these are slightly dated, they are still good references, nonetheless.

@ecruse - Were you able to find an actual solution to this? Having the same issue. Lacerte support on hold for over 2 hours! Thanks!
Unfortunately not. I spoke with support and they told me I would have to just retype all the questions into Link or vice versa. I can't recall if they gave me a specific reason, but I do remember the girl being almost as confused as I was. Another thing I found while using the Link application while in Lacerte was that you aren't able to rearrange the questions. So if you skipped a question while creating the questionnaire, you would have to overwrite an already typed Q with the skipped one, then re-type (or copy paste) every Q following where the skipped one is supposed to go. That or have your Qs out of order. Very non-user friendly in my opinion. We decided not to even use Link this year. Maybe next year if some kinks are worked out.
I am having the same issue. I do not like the standard questionaire and spend a lot of time creating a custom one every year. The instructions say that LINK will pull the questionaire based on the organizer settings, but not so. I have not found the solution. Support was no help. The drop down contains my stuff from 2017, not the current 2019 items.
