Thank you for responses over the weekend.  E Services Help desk was the answer, after the ordeal of getting through to them.  Apparently on the EFIN application, not only do you need to apply to e-file the 94x series, but there is a separate application area for "reporting agents".  The agent did this for me, I just need to log in and finish the "pending" submission process.  Then......it appears a 5 digit number will be issued and mailed for use with Intuit.
