You can enter a W-2, then at the top is Less Common Scenarios and at the bottom of that is 4852 to enter how you created this Substitute W-2 and how much work went into getting the information. If you don't have federal or state id numbers, it will have to be paper filed.
You can request a wage and earnings transcript from IRS https://www.irs.gov/individuals/tax-return-transcript-types-and-ways-to-order-them have the client order it online https://www.irs.gov/individuals/get-transcript
