If an extension is generated by Profile checkbox and e-filed, how do we know what was on it so we can include in the filed 1040? Is it was in the return when e-filed?

As long as there are no overrides on the extension screen this info will flow to Form 4868. You can always check your letters or Form 4868 to see what was submitted to the agency.
