Maybe it just goes as "Other Income"

Is it an addback for CT?
If so, either the CT wages box would be different than federal wages Box 1 or it would be a state tax "pickup" (that's what MD calls it)
I've never seen this be taxable to CT. It's Teacher Retirement Plan. A 414(h) doesn't qualify for the Federal Retirement Savings Credit. That's why we would want the answer to: what are you trying to do with it?
It is for a teacher. I know that in NYS it is taxable. I also have another one of those in Florida but the Florida one says FRS 3%. In my job here in Florida, I was told to enter it the software we use with 414h. Where can I find out if CT and FL are not taxed.
FL has no personal income tax.

Because NY taxes it "going in" and not when paid out. Is this CT person subject to NY taxes for a CT retirement?
