If the program doesn't have the GA 9465, you'll have to do it through the GA website.

The form is not available in PTO. Do as Lisa suggested.

FYI, you can check the availability of forms either within PTO using the link on the left panel from the homepage or on Intuit's website at https://lfs.intuit.com/.
