My client received Paid Family Leave which doesn't withheld tax, so the penalty.
I am filing the form to waive it, and on the instructions it says attached supporting documents. What kind of documents are needed? is doctor's note sufficient?

Choosing not to have withholding on PFL, or unemployment isn't an excuse to waive the underpayment penalty. You're wasting your time.
It is not choosing... PFL doesn't withhold tax by default, and my client doesn't know that. so the tax was not intentionally underpay.
By the definition, if it is not intentionally underpay, we can request a waiver. it is only an extra page to file with no additional cost. I don't see the reason not to try it and save some money.
