Charity gifts - 709 Form

I understand that if required to file a 709 Gift Tax form the IRS requires the taxpayer to include all gifts to charities on the return.  

Do I need to add every single charitable organization I donated to (as a donee)? Or would it be okay to add one (just one donee) with the a description that read "Charitable contributions made" or "Charitable gifts".

I believe it's all gifts to charities that are over the annual exclusion amount. Then you do need to list each charity as a donee.

The more I know, the more I don't know.
