9918 x .009 = $89 and is the appropriate addl withholding when wages are >$200K I believe.
Taxable wages are $204956
Medicare wages are $209918
Medicare withholding is $3133
Yes math was followed on 8959. Math is correct.
However, even the first line in Part V says "If you have more than one W-2....." and I believe the employer is suppose to take care of the over-withholding correction if it is just one W-2 as it is in this situation.
I just am not sure if it is appropriate to claim the additional Medicare tax withholding when there is only one W-2.
Read up on Additional Medicare Tax and the Form 8959.
Above a certain amount of wages, additional Medicate tax is withheld and due.
This has been around for several years.
209,918 x 1.45% = 3,044.
It appears that the one employer did overwithhold by $ 89. THAT should be refunded by the ER.
It has nothing to do with the F.8959.

Glad this is solved.... BUT where did F.8959 come from and is it now gone??
It should be there.
Even if this person is not subject to the additional tax - i.e. they file a joint return and spouse has no wages or SE income, or the joint total is <$250K- you show the add'l withholding on the 8959 and it counts as addl federal withholding on the 1040,
