Solution:
CIFT-620 Instructions include: The Louisiana Revenue Account Number must appear on each page of the return. Failure to provide your Revenue Account Number will result in an assessment for negligence penalty. The Federal Employer Identification Number (FEIN) cannot be used in place of the Revenue Account Number.
