To access the EF info worksheet:

Professional- 
Go to Forms > Select Form > (In the Open Forms window that pops up) type "EF" and it will automatically select. 
Hit Enter to open

Basic-
Click the "Find Forms" button > (In the Open Forms window that pops up) type "EF" and it will automatically select. 
Hit Enter to open
