You have likely associated depreciation with different activities. Look at the 2nd entry for depreciation "Form" if you are using different selections there, or in the next entry for "Activity Name or Number" that will generate multiple 4562. https://proconnect.intuit.com/community/federal-taxes/help/entering-depreciation-for-new-clients/00/...
