DON'T MISS THE CHECK BOX AT BOTTOM 

If you miss the check box at the bottom "Responsible party is authorized to open the Trump Account for the child" the form won't complete with the child info.

I have one with two kids and didn't see that I missed that box on one. I couldn't for the life of me get the second child to show on the form.

Of course, being buried here I should know haste makes waste: I slowed down and carefully went over the form line by line. So this is your heads up not to make the same mistake.
